Participating hospitals commit to one of three portfolios - Silver, Gold, or Platinum.
Every hospital commits to implementing all interventions in the basic Silver level
list:
- Acute myocardial infarction evidence-based care (AMI)
- Central line-associated blood stream infection prevention (CL-BSI)
- Congestive heart failure evidence-based care (CHF)
- Medication reconciliation (MedRec)
- Rapid response team deployment (RRT)
- Surgical site infection prevention (SCIP)
- Ventilator associated pneumonia prevention (VAP)
Participation at the intermediate or Gold level includes selection of one of these three additional options:
- Methicillin-resistant staphylococcus aureus (MRSA) reduction, and high-alert medication harm prevention, and hospital acquired pressure ulcer prevention (HAPU), or
- Stroke evidence-based care, or
- Sepsis prevention
Platinum or advanced level hospitals select two of the above options.
