Lucile Packard Children’s Hospital created standardized documentation in the electronic health record for
the RNs to pass on critical information and they changed the process of RN to RN hand-offs to ensure that
safety checks are completed.
- The ideal hand-off includes 2 nurses seeing the patient at the same time; this promotes teamwork rather than ‘he said, she said’
- Involve family and caretakers in the rounding process; who knows the patient better?
- Customize the hand-off process on individual units; one size doesn’t fit all
- Hand-offs should not be interrupted if they are to be truly effective

Staff nurses at San Francisco General Hospital influenced their peers to effectively implement an hourly rounding
program displaying how peer-to-peer influence can improve the care patients receive.
- Call it ‘rounding with purpose’ rather than referring to this as ‘hourly rounding’, brings meaning to the program
- Know your patient population and target rounds to greatest patient need
- Listen to the people who do the job; use staff nurses to collaborate across units to communicate and influence peers
- Involve the patient in falls prevention; think of the patient as the ‘mustard’ that binds the oil and vinegar
- Don’t waste resources that don’t result in improvement (e.g. ‘sitters’ for patients at high risk for falls)
- Involve physicians in post-fall huddles
- Culture interferes with testing

stories “from the field” about how nurses struggle with ineffective processes and work environments.
- Only 30% of a nurses time is actually spent at the bedside
- Storytelling is an effective tool to level the playing field
- A patient’s perception of engagement is often different from the staffs’ perception
- Identify the things that are taking nurses away from the bedside
